What Is Cialis (Tadalafil), and Is It the Same as Tadalafil?

Cialis is a brand name; tadalafil is the active drug inside it. In the United States, FDA-approved generic tadalafil products must demonstrate bioequivalence to the reference drug. That means an approved generic is expected to deliver the same active ingredient to the body in a comparable way and produce the same clinical effect when used as directed. Tablets may still differ in color, shape, markings, manufacturer, or inactive ingredients.

It is therefore misleading to describe Cialis and generic tadalafil as two unrelated medicines. They contain the same active ingredient, although a person with an allergy or sensitivity to an inactive ingredient may need a specific product. What Is Cialis Used For?

Under the current U.S. prescribing information for Cialis, the medicine is indicated for erectile dysfunction (ED), the signs and symptoms of BPH, and men who have both conditions. The reason one drug can be used in these different settings is that PDE5 signaling affects smooth muscle and blood-vessel regulation in more than one part of the lower urinary and reproductive systems.

Erectile dysfunction means repeatedly having difficulty getting or keeping an erection firm enough for sexual activity. Cialis (Tadalafil) can improve the part of an erection that depends on smooth-muscle relaxation and penile blood flow. It may help a man obtain greater firmness, maintain it longer during sexual activity, or respond more consistently when aroused. It does not cure atherosclerosis, reverse diabetic nerve damage, restore nerves injured during surgery, raise testosterone, or resolve relationship stress and performance anxiety.

A weak or inconsistent response does not automatically mean the drug is defective. Men should not increase the dose, repeat it sooner, or combine it with another ED medicine on their own. Why Cialis Can Work for Up to 36 Hours

The current prescribing information reports a mean terminal half-life of approximately 17.5 hours in healthy adults. A half-life is the average time required for the measured amount of a drug in the body to fall by half; it is not the length of an erection and it does not mean that everyone experiences the same duration.

Because tadalafil leaves the body relatively slowly, an as-needed dose may continue to support erectile responses for up to 36 hours in some men. Each erection within that period still requires sexual stimulation, and it should subside normally after stimulation ends. This is what the 36-hour Cialis window means: an opportunity for a new response, not a single continuous erection.

The remaining drug concentration changes gradually, while the clinical response varies with dose, age, kidney and liver function, other medicines, and individual physiology. A prolonged therapeutic window is not permission to take extra tablets. The prescribed interval still applies even if no sexual activity occurs after a dose.

From IC351 to Cialis: How Tadalafil Was Developed

Tadalafil was not the product of a single named inventor working alone. A peer-reviewed history of tadalafil development describes a research path that began with a collaboration between Glaxo Wellcome and ICOS. ICOS began studying the compound IC351 in 1993, Phase I trials started in 1995, and studies involving men with ED followed in 1997. When Cialis May Not Be Safe

The most important absolute contraindication is concurrent nitrate therapy. Tadalafil(Cialis) can amplify the blood-pressure-lowering effect of nitroglycerin, isosorbide, and other organic nitrates enough to cause dangerous hypotension. The label also contraindicates use with guanylate cyclase stimulators such as riociguat. Recreational “poppers” may contain nitrates and create the same type of risk.

Alpha-blockers and antihypertensive drugs are not identical to nitrates, but combining them with tadalafil may further lower blood pressure. Some strong CYP3A4 inhibitors can raise tadalafil levels and require a different dose or interval. Taking tadalafil together with sildenafil, vardenafil, avanafil, or another tadalafil product has not been established as safe and should be avoided unless a qualified prescriber has explicitly directed otherwise.

Health Conditions a Prescriber Needs to Consider

Sexual activity itself places a temporary demand on the cardiovascular system. A clinician may need to assess whether sexual activity and ED treatment are appropriate after a recent heart attack or stroke, with unstable angina, uncontrolled blood pressure, serious rhythm problems, heart failure, or other significant cardiovascular disease. Kidney or liver impairment can change tadalafil exposure and may limit the permitted regimen. A history of priapism, certain penile conditions, bleeding disorders, retinal disease, sudden hearing loss, or severe medication reactions also belongs in the review.

Why Cialis Requires a Prescription in the United States
